Waiver of Liability/Hold Harmless Agreement
The Waiver of Liability/Hold Harmless Agreement (following page) must be signed and dated by both the guest and the sponsoring member each time a guest visits the range. Place the signed form in the receptacle provided in each range area.
Members must sign the form annually as part of their New Member Application or annual Renewal Form. Both spouses must sign if both plan to use the range, and the names of any minor children accompanying them to the range must be listed.
How to fill out the Waiver form:
If any minors (under 21) will be accompanying you to the range, enter their names, ages, and relationship to the primary member. Enter their address if it differs from that of the primary member. Enter an emergency contact name and phone. If more space is needed for additional minors, use the back of the form.
Enter the name of any adult participant who is not the primary member. Indicate whether the adult is the spouse of the primary member or a guest. The spouse or guest must sign their full legal name. If the adult is a guest, enter their address. Use a separate form for each guest.
Enter the name and full legal signature of the primary member.
